About this file

This is a Drug-Free Workplace Certification Form required by the Hillsborough County Board of County Commissioners in Florida for the Cat Litter supply contract opportunity. Vendors responding to this solicitation must complete and sign this certification form as a mandatory compliance requirement under Florida Statute 287.087. The form applies to all employees engaged in providing commodities or contractual services under the bid, and vendors must certify their commitment to maintaining a drug-free workplace throughout the one-year contract term valued at $37,500.00.

The certification requires vendors to implement six specific compliance measures: publish a statement prohibiting unlawful manufacture, distribution, dispensing, possession, or use of controlled substances in the workplace; inform employees of drug abuse dangers and available assistance programs; distribute the policy statement to all relevant employees; require employees to notify the employer of drug-related convictions within five days; impose sanctions or require participation in rehabilitation programs for violations; and demonstrate good-faith efforts to maintain workplace drug-free compliance. The authorized company representative must sign and date the form, affirming that the firm fully complies with all stated requirements.
