About this file

This is a Drug-Free Workplace Certification Form required by the Hillsborough County Water Resources Department in Florida for the Hydra-Stop Insta-Valve 250 Patriot Insertion Valves contract opportunity, valued at $175,000.00. The form mandates that all vendors bidding on this project comply with Florida Statute 287.087 by establishing and maintaining a drug-free workplace policy. Vendors must publish statements notifying employees that unlawful manufacture, distribution, dispensing, possession, or use of controlled substances is prohibited in the workplace and must specify the actions taken against employees for violations. The form does not specify particular response deadlines, site visits, or bidders' conferences; rather, it serves as a prerequisite certification document that must be completed and signed by an authorized representative of the bidding firm.

The form requires vendors to inform employees about the dangers of drug abuse, maintain workplace policies promoting drug-free environments, distribute the required statement to all employees engaged in providing the commodities or contractual services under bid, and notify employees of their obligation to report any convictions or guilty pleas related to controlled substance violations within five days of the conviction. Vendors must impose sanctions or require participation in drug abuse assistance or rehabilitation programs for employees found in violation and make a good faith effort to maintain a drug-free workplace. This certification form represents a compliance requirement rather than a pricing or funding document and must be executed by the offeror's authorized signature before bid submission.
