PERFORMANCE WORK STATEMENT

351 Special Warfare Training Squadron, Mobile Latrine, Service Support Kirtland AFB, NM 87117

6 April 2021

1. The 351 Special Warfare Training Squadron (351 SWTS) conducts training in the state of New Mexico and requires portable latrines, portable hand wash stations and/or portable showers in order to meet Federal and State land use agreements. The training locations require suitable portable latrines, portable hand wash stations and/or portable showers that can be staged before training starts and pick-up when training is complete.

The training locations include Kirtland Air Force Base, Del Norte Gun Range, Grants, and Belen.

2. The contractor shall appoint a Point of Contact (POC) who will have responsibility for the performance of work. The Point of Contact will also have authority to act for the contractor on all matters related to the contract.

3. Training is performed up to 3 times per year and lasts up to 2 weeks at a time, depending on the specific location. The 351 SWTS will notify the contractor no later than five (5) business days prior to requiring service. This notification will identify the training location from among one of the five (5) categories below. The contractor will furnish all labor, tools, parts, materials, facilities and transportation to provide these services. The contractor will deliver the portable latrines, portable hand wash stations and/or portable showers to the training location no later than 0700hrs on the first day of training and pick-up/transport the portable latrines, portable hand wash stations and/or portable showers from the training location no earlier than 1700hrs on the last day of training.

Servicing must be performed within OSHA regulations. Each latrine must be fully stocked with four toilet paper rolls. Requirements, including durations, portable devices provided, and locations for each training event shall include:

Del Norte Gun Club Transport, deliver and pick-up six (6) portable latrines and two (2) hand wash stations to the Del Norte Gun Club training location on the first day of training; Up to three (3) times per year, six (6) days for each session

Grants, NM Transport, deliver and pick-up one (1) portable latrine to the Grants, NM training location (35° 9.546'N, 108° 6.340'W) on the first day of training; Up to three (3) times per year, three (3) days for each session

Belen, NM Transport, deliver and pick-up four (4) portable latrines and two (2) hand wash station to the Belen, NM training location (34° 38.810'N, 106° 49.638'W) on the first day of training; Up to three (3) times per year, three (3) days for each session

Kirtland AFB Transport, deliver and pick-up four (4) portable shower stations to the training location on Kirtland AFB, NM; Up to three (3) times per year, fourteen (14) days for each session. One (1) trip for servicing of the portable shower stations shall be included for each training session.

Please note, access to the training locations will require off-road, all weather type vehicles.

4. The contractor will be licensed and insured. No Security Clearance is required. The contractor shall perform all phases of the work from start to completion and coordinate with the designated POC before starting work. Below are personnel authorized to place calls against services:

5. The contractor will protect all government and personnel property from damage. Any damage caused by the contractor will be replaced or repaired at contractors cost prior to payment. Contractor will abide by all OSHA Regulations, concerning proper wear of Personal Protective Equipment (PPE), and Material handling Equipment (MHE) and lifting techniques.

6. A final inspection of the completed work will be conducted between the contractor and an appointed representative of the 351 SWTS prior to payment. All discrepancies noted during the final inspection or during the final acceptance inspection shall be corrected prior to invoice and payment. The area must be completely cleaned and returned to its original state. All inspection/repair documentation will be provided to the 351 SWTS representative.

7. The contractor shall only invoice based on the actual services provided (i.e. if less than the maximum number of latrines, hand wash stations, or showers are requested, the invoiced amount shall be proportionally lower).
