About this file
This document is an Attachment IV form related to Texas Senate Bill 252, applicable to governmental contracts within the City of Texas. The form requires offerors to disclose and confirm they do not have business relationships with Iran, Sudan, or foreign terrorist organizations as defined by the Texas Comptroller of Public Accounts.
The legislation, effective September 1, 2017, prohibits the City from entering into governmental contracts with providers identified on the comptroller's list as doing business with specified entities. Companies that the United States government has affirmatively declared as excluded from federal sanctions regimes related to Sudan, Iran, or foreign terrorist organizations are exempt from these contract prohibitions. The form requires an authorized representative to sign and acknowledge they are not on the restricted list, serving as a compliance mechanism for government contracting.
