About this file

This is an Anti-Human Trafficking Affidavit required by the State of Florida for a contract opportunity involving Operations of the Biosolids Dewatering Facility, Sludge Dewatering, and Hauling. The affidavit must be executed, renewed, or extended with every contract and requires the contractor to attest under penalty of perjury that the entity does not use coercion for labor or services. The document outlines seven specific definitions of coercion prohibited under Florida Statute 787.06(13), including physical force, unlawful restraint or confinement, exploitative debt practices, confiscation of identification documents, threats of financial harm, fraud or deception, and provision of controlled substances for exploitation purposes.

The affidavit requires an authorized representative of the contracting entity to provide a signature, printed name, title, entity name, and date. This certification is a mandatory compliance requirement for all contractors bidding on or executing state and local contracts in Florida and serves as an anti-trafficking protection measure to ensure ethical labor practices throughout the contract term.
