About this file
This document is an Addendum No. 2 for a Request for Proposal (RFP) issued by the Florida Department of Transportation (FDOT) for District Four, specifically for the Districtwide Implementation & Maintenance of National Pollutant Discharge Elimination System (NPDES) Permits. The RFP (DOT-RFP-25-4004-SP) has an opening date of 7-24-2025 and includes revised Exhibit A - Scope of Services and Exhibit C-1 - Price Proposal Form, both dated 7/17/25. The addendum addresses vendor questions and provides detailed personnel qualifications for various positions including Project Manager, Environmental Specialist, Senior Scientist, Scientist, Inspector, Engineering Intern, GIS Specialist, and Clerical staff.
The addendum clarifies that all unit rates are loaded rates inclusive of salaries, overhead, operating margin, and direct expenses. It also highlights specific personnel requirements, such as minimum education levels (typically a Bachelor of Science or Engineering), required years of experience (ranging from 1-20 years depending on the role), and mandatory certifications like FDEP qualified Management Inspector. The document notes a discrepancy between Exhibit A and Exhibit C regarding position classifications, specifically the removal of Senior Inspector and addition of Engineering Intern, with no changes made to the Price Proposal Form. Bidders are required to acknowledge receipt of this addendum by returning it to the Procurement Office via email by the proposal opening date, with potential disqualification for failure to do so.
