About this file

This is a Bid Form and Price Schedule (Attachment A) for Murray City, Utah's Invitation for Bid (IFB) 26-22 for the Park Center Roof Replacement project. The bid form requires contractors to submit a total base bid covering all work across nine designated roof areas at the Murray Park Center, including labor, materials, equipment, demolition, disposal, permits, fees, temporary facilities, site protection, cleanup, warranties, insurance, bonding, overhead, and profit. The project scope encompasses removal and disposal of existing roofing materials, deck and substrate repairs, insulation installation, and placement of a new 60 mil TPO roofing system with associated drainage improvements, flashings, coping, parapet work, and related accessories. Bidders must acknowledge all addenda received with their submission and provide an authorized representative's signature, title, and date.

The pricing structure includes a Total Base Bid used solely for bid comparison and award determination, separate unit prices for optional items including insulation replacement, steel deck replacement, coping, wood nailers, and walk pads quoted per square foot or linear foot, and two additive alternates for ladder access installation and release-sheet TPO system installation that may be elected by the City after award subject to available funding. Bidders must complete a Construction Preference Certification addressing six preference categories based on Murray City Code 3.10.370, including qualified health insurance coverage, drug and alcohol testing policies, veteran programs, job training programs, safety programs, and formal nondiscrimination policies, with supporting documentation required for each claimed preference. Award remains contingent upon responsiveness, responsibility determinations, and verified construction preferences applied during evaluation.
