About this file

This document is a Worker Protection and Investment Certification Form for a state and local contract opportunity in Pennsylvania. The Commonwealth of Pennsylvania, Department of Conservation and Natural Resources (DCNR), Tiadaghton State Forest, requires services to deliver and paver-place approximately 5,050 tons of sandstone, limestone, or basalt Driving Surface Aggregate (DSA) to Robbins Road, Crawford Township, Clinton County. Contractors and grantees must certify that they are in compliance with various Pennsylvania state labor and workforce safety laws, including the Construction Workplace Misclassification Act, Minimum Wage Act, and Prevailing Wage Act. Failure to provide accurate certification can result in contract termination and three-year ineligibility to bid on future contracts.

The certification form requires the contractor/grantee to confirm their compliance with the applicable state labor and workplace safety laws. The contractor must immediately report any change in their compliance status to the Purchasing Agency. The certification is subject to the provisions and penalties of Pennsylvania's Unsworn Falsification to Authorities statute.
