About this file
This Request for Proposal (RFP) is issued by the School Board of Polk County, Florida Purchasing Department seeking proposals from qualified firms to provide a voluntary, fully insured Group Dental Plan for the District's active employees, dependents, retirees, and COBRA participants. The District currently employs approximately 12,000 employees and has offered three voluntary Group Dental PPO Plans through Delta Dental since January 1, 2011. Proposers must submit sealed proposals by 3:00 p.m. Eastern Time on May 8, 2015, with written questions due by April 17, 2015. The District will conduct an evaluation process with potential finalist presentations scheduled for June 5, 2015, and Board approval anticipated on or about June 23, 2015, or July 28, 2015, depending on the Board meeting calendar. The initial contract period will be three plan years beginning January 1, 2016, with options for additional renewal periods subject to mutual agreement. The District requires minimum participation waivers, acceptance of self-billing practices with monthly payroll remittance, weekly eligibility file submissions, and comprehensive member services including educational materials, identification cards with unique identifiers, and quarterly and annual performance reports.
Proposers must submit premium rates on a four-tiered basis with a mandatory two-year fixed rate for years 1 and 2 (2016-2017) and a fixed rate, flat rate, or rate cap for year 3 (2018). An administrative fee of $1.00 per subscriber per month, paid to the District, must be included in the premium proposal. The District prefers net-of-commission pricing but does not prohibit agent commissions. Current monthly premiums for the three plans range from $12.58 to $129.19 depending on coverage tier and family status. Proposals are evaluated on a maximum of 500 points for written submissions, with possible additional 100 points for oral presentations if required, using criteria including General and Administrative (75 points), Plan Design (125 points), Network (125 points), and Premium Proposal (175 points). The premium evaluation uses a comparative scoring formula based on the lowest three-year combined total annual premium submitted. Proposers must provide proof of required insurance coverage, including $1,000,000 commercial general liability, $1,000,000 professional liability, and workers' compensation at statutory limits, with the Board named as additional insured. The contract is contingent upon budget appropriation and successful negotiation of terms acceptable to both parties.

The District will not be under any requirement to complete evaluations by any specific date and reserves the right to suspend or postpone the evaluation process should the need arise due to budget constraints, time constraints or other factors. However, it is anticipated that the review/evaluation process will be completed in a timely manner. Network 125
*Premium Proposal 175
Total Points 500
*Calculation of the premium proposal: Points for the “premium proposal” shall be determined by using the total annualized premium for the low, middle and high plan options combined, based on the counts provided, for the three year contract period. This will represent a 3-year combined total annual premium. Distribution of points for evaluation of Premium Proposal will be calculated using the following formula:
Lowest 3-year combined total annual premium ÷ Proposer’s 3-year combined total annual premium X Total Point Value = Proposer’s Premium points
For example, if Respondent A submits a proposal with a 3-year combined total annual premium of $10,000 and Respondent B submits a proposal with a 3-year combined total annual premium
$15,000 and Respondent C submits a proposal with a 3-year combined total annual premium
$12,500, Respondent A would receive 100% of the possible points, Respondent B would receive
67% ($10,000/$15,000) of the possible points, and Respondent C would receive 80%
($10,000/$12,500) of the possible points.
