About this file
This is an Affirmation and Disclosure Form required by the State of Ohio for contractors bidding on the installation of a security system for three buildings at Fernwood State Forest. The form mandates that contractors disclose all locations where services will be performed and where state data will be located. Site visits are available by appointment only between January 6, 2026 and January 23, 2026, and interested parties should contact Ryan Zeisler at 740-381-1882 to schedule. The contractor must affirm compliance with Executive Orders prohibiting offshore service performance, locating state data offshore, and purchasing from Russian institutions or companies.
Contractors and their subcontractors are required to provide their principal business locations and the specific addresses where contract services will be performed and where state data will be stored. The form stipulates that contractors have a continuing duty to disclose any changes or shifts in service location before, during, and after contract execution and must notify the State immediately of any such changes. The State retains the right to immediately terminate the contract if services are performed outside the United States unless the State provides a duly signed waiver. A duly authorized representative of the contractor must execute the form, and it becomes an incorporated part of any resulting contract with the State.
