About this file

This document is a Statement of Work (SOW) for preventive maintenance and repair services for two GE Carestation 620 Anesthesia Machines located at the Whiteriver Indian Health Service (WRSU) Hospital in Whiteriver, Arizona. The SOW requires the contractor to provide all supervision, labor, parts, materials, tools, supplies, maintenance, calibration, transportation, and equipment service for the anesthesia machines, including certifications, calibrations/calibration verifications, and replacement parts as needed. The work must be performed according to the original equipment manufacturer's (OEM) specifications, as well as relevant NFPA, Joint Commission, and regulatory standards. The contractor must have OEM authorization and the necessary licenses/certifications to perform the services. The work is to be completed during normal business hours, Monday through Friday, with a 1-business day response time for repair requests. The contractor must submit detailed service and repair reports within 15 calendar days of completing the work.

The related federal contract opportunity is a request for quotation (RFQ) for the preventive maintenance and repair services described in the SOW. This is a 100% small business set-aside solicitation conducted under the authority of FAR Part 12, Acquisition of Commercial Products and Commercial Services. The Phoenix Area Indian Health Service anticipates awarding a single firm fixed-price purchase order for these services.
