About this file
This is an RFP Evaluation Plan (Amended) for Medical Supplies issued by the State of Idaho through the NASPO ValuePoint cooperative purchasing program (RFP 1433). The solicitation encompasses three categories: Category 1 (Consumables and Supplies across 14 subcategories with 100 line items), Category 2 (Equipment and Devices across 14 subcategories with 20 line items), and Category 3 (Support and Maintenance, available only to vendors awarded Category 2). Offerors may propose for one or both product categories and must supply all products within their selected category and offer discounts for all subcategories. The pre-proposal conference is scheduled for October 7, 2026 at 11:30 a.m. Mountain Time via video conference, with questions due October 14, 2026 at 11:59:59 p.m. Mountain Time. Proposals are due November 9, 2026 at 5:00 p.m. Mountain Time, followed by public bid opening at 10:30 a.m. Mountain Time the next business day via Microsoft Teams. The initial contract term is one year with options to renew for up to seven additional one-year periods, for a maximum of eight years total.
The evaluation process consists of four stages: Stage 1 (Initial Responsiveness), Stage 2 (Mandatory Minimum Requirements), Stage 3 (Technical Criteria evaluated on 500 points covering Organization, Staffing, and Experience at 200 points; Scope of Work at 100 points; Warranty, Customer Service & Technology at 100 points; and Cooperative Contracting Ability at 100 points), and Stage 4 (Cost Evaluation on 500 points for category discounts). Technical proposals scoring below 300 normalized points are rejected. Cost points are awarded proportionally based on the highest total average percentage discount offered. Awards will be made to the lowest responsible bidder by category, with the Lead State reserving the right to make multiple awards per category. Contractors must pay NASPO ValuePoint an administrative fee of 0.4 percent of invoiced sales (0.33 percent if fully compliant with reporting) quarterly and maintain minimum commercial general liability insurance of $1 million per occurrence and $2 million general aggregate. Prior to award announcements, the Lead State will present recommendations to NASPO ValuePoint for approval.

Evaluation Overview Offerors proposing solutions for both Category 1 (Consumables and Supplies) and Category 2 (Equipment and Devices) will be considered for an award separately for each Category. Offerors must be able to supply the products and offer a discount for all subcategories listed under each Category (1 & 2). Any such Offeror's technical response in Attachment H must address both Categories. The Offeror's Attachment H will be evaluated and given a raw score, which will then be normalized as per Stage 3: Technical Criteria Evaluation. The normalized technical score rankings will be applied separately per Category and combined with the Category's cost evaluation score to result in the final score.
The Offerors will not receive any additional points for proposing pricing for both Categories 1 and 2.
Category 3 (Support and Maintenance) will be awarded to Offerors awarded in Category 2.
Stage 1: Initial Responsiveness Evaluation. Proposals will be reviewed for completeness and initial responsiveness. Proposals omitting required documents or responses may be rejected in accordance with Attachment A, RFP Terms and Conditions. Note: IDAPA 38.05.01.074.03 and 38.05.01.091.05 allow the designated State official to waive minor informalities as well as minor deviations.

The scores for the Technical Proposals will be normalized as follows: The Technical Proposal with the highest raw score in the applicable Category, will receive all available Technical Points: 500 points. The remaining Proposals in that Category will be assigned a proportional amount of the available Technical Points using the formula:
| 500 X | Raw Score of Technical Proposal Being Evaluated | |
| Highest Raw Technical Score |
Stage 4: Cost Evaluation. Cost Proposals will only be reviewed following completion of the Technical Proposal evaluations. Cost Proposals will only be evaluated for Offerors that meet the minimum technical score requirement detailed in Stage 3.

| Stage 3 Total: |
| 500 |
Points for the Category Discount evaluation will be normalized as follows: The Cost Proposal, with the highest Total Average % Discount for the Category Discounts in the applicable Category will receive all available Cost Points: 500 points. The remaining Proposals in that Category will be assigned a proportional amount of the available Cost Points using the formula:
| 500 | X | Lowest Total Cost Total Average Discount Being Evaluated |
| Total Cost Being Evaluated Highest Discount Average |
The Lead State reserves the right to assign a nominal value to “0” cost values if application of the formula results in an error, negative numbers, or an unreasonably skewed distribution of points.
Award Selection Award will be made to the lowest responsible bidder(s), defined as the high point responsive responsible Offeror(s), by Category, in accordance with the Evaluation Method described above. The State may make multiple awards for each Category, in accordance with Idaho Code section 67-9211, at its discretion; and may choose not to award one or more Categories, in the best interest of the State.
Prior to announcement of awards and execution of Master Agreements, the Lead State will present an award recommendation to NASPO ValuePoint for approval of the proposed awards.
Following approval of NASPO ValuePoint, Idaho will issue notices of intent to award contract(s) to the winning vendors.
