About this file

This is a Price Response form (Attachment 3) from the Department of Financial Services for RFP COS 2425-09. The form requires respondents to provide pricing for two deliverables: a Draft Report and a Final Report, which must be itemized individually and summed for a Total Projected Price.

The form also includes an optional pricing table for additional elements, which will not be considered in the price point evaluation. These optional elements consist of estimating the impact of a 1-in-100 year hurricane on State-owned buildings and providing recommendations for reducing the economic impact from such an event. Respondents are not required to complete the optional pricing table.
