EXECUTIVE SUMMARY

Mabbett and Associates, Inc. (M&A) performed the following at Albany VAMC in accordance with the VISN 2 scope of work:

• Completed comprehensive asbestos inspections by New York State licensed asbestos inspectors in accessible areas of 11 buildings.

• Performed labeling of asbestos containing thermal system insulation such as piping and boiler insulation and provided CADD plans showing asbestos locations for posting and notification purposes by the VAMC.

• Prepared building reports with results, tables, and CADD building plans showing asbestos locations.

• Prepared an Asbestos Operations & Maintenance Plan in accordance with the August 2010 VHA Asbestos Management Directive.

Visual inspections and sampling were performed in general accordance with U.S.

Environmental Protection Agency (EPA) Asbestos Hazard and Emergency Response Act (AHERA) protocols. This report is not intended to be used to generate bid documents, specifications, etc. for asbestos abatement projects or renovation or demolition projects.

Significant findings from the asbestos inspections included:

• A total of 904 samples were collected for asbestos analysis. Materials sampled included floor tiles, mastics, ceiling tiles, thermal insulation, caulking, and other miscellaneous suspect asbestos containing materials.

• 8 of the 11 buildings surveyed had asbestos containing materials. (See Table 4)

• Approximately 88 linear feet of pipe insulation containing asbestos was identified which is in need of repair or abatement by properly trained/licensed contractors in the near term if not already corrected due to damaged condition. (See Table 5)

• The total estimated current liability if the VAMC was to abate all the asbestos containing materials identified is over $1.5M. The VAMC is required to report the asbestos liability estimate quarterly. (See Table 7)

This asbestos assessment report consisting of four volumes is considered a living document that will require on-going, periodic revisions and updates to ensure accuracy as the status of the asbestos and building conditions change over time.

The assessment should serve as a cornerstone for a facility asbestos program that should be carefully maintained to comply with Federal Regulations, protect human health and serve as a useful tool when evaluating planned or unplanned building material disturbance.

Implementation and effective on-going management of the asbestos program requires a team approach and involvement from senior management, Safety, Engineering, Facilities, Housekeeping and others. It is recommended that Medical Center Directors assign qualified individuals, including Asbestos Program Managers, to coordinate and oversee asbestos impacting activities to ensure effective management of on-going revisions of this facility asbestos survey, asbestos removal and maintenance activities.

This comprehensive report, including individual Building Reports (Volume II), is the product of the M&A’s effort. The Report consists of four volumes: Volume I – Executive Summary & Introduction; Volume II - Individual Building Reports, containing floor plans indicating ACM sample locations with findings information, and survey findings for the specific surveyed building;

Volume III – Appendices/Supporting Data, including laboratory results and certifications; and Volume IV - the ACM O&M Plan.

Purpose

Visual inspections and sampling were performed in general accordance with US Environmental Protection Agency (EPA) Asbestos Hazard and Emergency Response Act (AHERA) protocols. The purpose of this AHERA type asbestos inspection survey was to identify asbestos containing materials in areas that were accessible to the survey team. This inspection report and associated individual building reports are not intended to be used to generate bid documents, specifications, etc. for asbestos abatement project or renovation or demolition projects since samples were not collected behind walls, above hard ceilings, etc. and therefore additional sampling of suspected ACM in locations scheduled for demolition/ renovation may be required.

Summary of Findings

A total of 904 samples were collected for analysis. Materials sampled included floor tiles, mastics, ceiling tiles, thermal insulation, caulking, and other miscellaneous suspect ACM. Of the buildings surveyed for ACM, the buildings listed in Table 3 had no ACM identified based on the results of the surveys of accessible locations and analytical results for samples collected which were all reported by the laboratory as “No Asbestos Detected” (NAD) for these buildings.

See individual building reports for specific sample locations, building drawings, and results or Volume I – Chapter 2 for a table of all samples collected during this survey and any presumed ACM (PACM). PACM’s are materials identified as ACM in previous surveys or VISN 2 directed assumed materials which include original TSI in older buildings and 9”x9” floor tile and mastics.

A summary of all positive ACM and PACM identified during this survey is provided in the Volume IV:

All identified ACM was assigned a risk hazard category ranging from 1 (worst) to 4 (best). The ratings takes into account: condition, friable vs. non-friable, accessibility, occupancy (e.g.

continuous, intermittent or occasional and patients/staff/visitors), potential for air erosion, potential for vibration damage, potential for disturbance/damage (e.g. exposed and in an accessible location), and potential for water damage. Individual building reports and Volume 1

– Chapter 2 includes all of the associated risk hazard category ratings based on the following criteria:

Category 1: represents significantly damaged friable ACM in locations with moderate to high occupancy by staff and/or patients. A significantly damaged condition is a condition with damage greater than or equal to 10% of the total surface area or 25% localized damage. Category 1 ACM should be abated or repaired as soon as possible to minimize any potential hazards posed by the asbestos.

Category 2: represents significantly damaged or damaged friable ACM in locations not frequently occupied. Category 2 ACM should be abated or repaired as soon as possible to minimize any potential hazards posed by the asbestos.

Category 3: represents ACM in at least good condition (friable or non-friable ACM that could become friable). A good condition is a condition with damage less than or equal to 1% of its total area or less than or equal to 1% localized damage. Category 3 should be visually surveyed semi-annually as part of the routine O&M program and should be considered for abatement or corrective action if conditions deteriorate or circumstances allow.

Category 4: represents non-friable ACM in good or excellent condition but should be regularly inspected as part of the O&M Plan.

Asbestos Survey Methodology Summary

A visual inspection was conducted by New York State Licensed Asbestos Inspectors to determine the locations of suspect building materials in the buildings listed in Table 1.

Sampling and inspections were performed in general accordance with AHERA protocols. Only areas that were accessible during the field work phase were surveyed. Areas which were inaccessible were identified in the individual building reports. Suspect ACM material that may be present within the walls, above inaccessible hard ceilings, or in other inaccessible locations, that was not sampled should be assumed to contain asbestos if discovered during any renovation process or until otherwise verified. To avoid impacting any existing roof warranties, no destructive sampling of roofing materials was included in this survey, and suspect roofing material should be assumed to be ACM until sample results prove otherwise.

The bulk samples collected as part of this effort were submitted under chain of custody procedures to America Science, Inc. (Amerisci) of New York, NY for polarized light microscopy (PLM) and transmission electron microscopy (TEM) analysis as required by the New York State Environmental Laboratory Approval Program (ELAP). PLM analysis of friable materials was performed using NYS ELAP Method 198.1. Samples of Non-friable Organically Bound (NOB) materials were analyzed by PLM (NYS ELAP Method 198.6) subsequent to matrix

© 2011, Mabbett & Associates, Inc. Vol I, Chapter 1 – Page 6 of 8 reduction. NOB samples that were identified as non-ACM by PLM methods were re-analyzed by TEM (NYS ELAP Method 198.4) for confirmation purposes.

The PLM analytical protocol requires each layer of the sample to be analyzed separately. The quantity of analyses will vary based on the number of layers in a sample and whether a "positive stop" is employed. A positive stop (PS) is used when one sample of a homogeneous area is positive, the remainder of the samples were not be analyzed because the entire homogeneous area is considered positive as indicated on the results tables and drawings.

Amerisci is accredited by the American Industrial Hygiene Association (AIHA) and participates in the National Voluntary Laboratory Accreditation Program (NVLAP Lab Code 200546-0).

Copies of the laboratory accreditations are included in Volume III of the Comprehensive VAMC Asbestos Survey Report.

Previous Report Summary

The VAMC was asked to provide previous asbestos survey reports/laboratory results (Table 6). If provided, M&A reviewed reports during the survey, and used the information to identify locations where ACM materials were previously noted. By building upon the existing information, M&A attempted to improve the total number of samples collected.

O&M Plan

The Asbestos O&M Plan prepared by a New York State Licensed Asbestos Management Planner found in Volume IV of the Project Report satisfies the Occupational Safety and Health Administration (OSHA) and VHA Asbestos Management Program Directive compliance requirements for employee safety and for management of asbestos identified at the facility. This O&M Plan contains the asbestos survey results, background information on asbestos, an introduction to the O&M program, information on implementing the O&M program, O&M work practices, O&M incident and emergency work practices, and O&M surveillance and documentation practices.

During preparation for renovation activities, the VAMC should review the Asbestos O&M Plan and specific building reports to determine if asbestos-containing materials may potentially be disturbed by the proposed renovations. The VAMC should also assume materials not previously sampled due to inaccessibility and which will be impacted by renovation activities to be ACM unless otherwise determined. If ACM must be abated as a part of the renovations, ACMs must be removed by a New York State Licensed Asbestos Abatement Contractor and proper notification to the New York State, Department of Labor, Asbestos Bureau must be

© 2011, Mabbett & Associates, Inc. Vol I, Chapter 1 – Page 7 of 8 made as required.

If any ACM is damaged or becomes damaged it should be repaired, if possible, or removed entirely. Materials classified with a high potential for disturbance (Category 1 and 2) should be removed or repaired immediately upon discovering damage to the material especially when located in occupied areas. Additional information regarding the O&M program for in-place ACM is available in Volume IV – O&M Plan.

Quarterly Liability Report

The VAMC must submit a quarterly ACM liability report. At the time of the survey, identified quantities of ACM were estimated/quantified and cost estimates for abatement were developed.

Identified ACM should be included and accounted for in the liability report until removed from the building. As outlined in the O&M Plan as abatements occur or other changes to the amount or condition of ACM are identified, the liability tables and associated building reports and CADD plans should be updated to reflect the current conditions and available information including current abatement market rates. Table 7 is a consolidated summary of the estimated liability for the VAMC based on identified ACM at the time of the survey. The estimated liability table by building and ACM type can be found in the Volume IV – O&M Plan Appendix F.

Conclusions and Limitations

M&A greatly appreciates all of the support provided by the VAMC to complete this survey and the opportunity to support Albany’s Asbestos Management Program. Based on the information provided in this complete report and consistent with the VHA directive and other regulatory requirements the VAMC should continue to implement an asbestos management program for the protection of employees, patients and visitors. Most importantly ACM which is damaged and poses a risk should be addressed as soon as possible to minimize the risk.

Information in the project documentation provided relating to ACM and conditions although believed to be inclusive and accurate, was based on limited assessments and representative field sampling. Limiting conditions included limited sampling and inaccessible areas such as between walls and floors of the structures as indicated in this report.

* - The VISN 2 risk hazard assessment scale 1 – 4 is a relative indicator of the risk and need for response/remediation. (1) represents the highest priority (e.g. removal or encapsulation) whereas a (4) represents the lowest priority (monitor as part of 6 month O&M program). The rating, assigned by an Asbestos Management Planner, takes into account the following criteria: condition, friable vs. non-friable, accessibility, occupancy (e.g. continuous, intermittent or occasional, patients/staff/visitors), potential for air erosion, potential for vibration damage, potential for disturbance/damage (e.g. exposed and in an accessible location), and potential for water damage.

Footnotes:

NAD - No Asbestos Detected (PLM) ND - None Detected (TEM) NA/PS - Positive Stop
