About this file
This is a Certification of Bidder/Contractor CDL Compliance Program document issued by the Brockway Area Sewage Authority in Pennsylvania for the Clarifier Bearings, Shaft and Skirt Replacement Project 2026 - R1. The project involves the removal and replacement of existing skirts in Clarifiers #1 and #2, as well as the replacement of the steady shaft and bearing in Clarifier #2 at the Brockway Area Sewage Authority Wastewater Treatment Plant. All contractors and subcontractors performing work on this project must ensure that employees performing safety-sensitive functions requiring a Commercial Driver's License (CDL) participate in a qualified drug and alcohol testing program. Prior to contract award and every six months thereafter, contractors and subcontractors must certify compliance with federal Department of Transportation regulations (49 CFR Parts 40 and 382) by completing and submitting the certification form included in the bidding documents.
The certification requirement mandates that contractors provide documentation of their participation in an approved drug and alcohol testing program and furnish the most recent DOT Form 1385 (Drug and Alcohol Testing MIS Data Collection) upon request by the Owner. Bidders must provide the testing program name, contact person, address, and phone number as part of their certification submission. The certification form requires the signature of an authorized representative of the contractor or subcontractor and must be dated prior to award. This requirement applies to all entities bidding on or performing work under the project contract.
