STATEMENT OF WORK

FELSENTHAL NATIONAL WILDLIFE REFUGE:

ABANDONMENT AND CLOSURE OF WATER WELLS

General Work includes the proper sealing, plugging, and abandonment (e.g. closure) of two electric, potable water wells located on Felsenthal National Wildlife Refuge (NWR), Crossett, Arkansas. Estimates must include all labor, fees, materials, transportation, documentation, etc. associated with this project. Additionally, all work shall be performed by a licensed water well driller, and in accordance with the most current regulations associated with proper well abandonment and identified by the Arkansas Water Well Construction Commission (Reference Attached).

Historical Information, Exploratory Investigation and Locations All visits to the refuge shall be scheduled in advance with:

Michael Stroeh - Project Leader Felsenthal NWR 5531 Highway 82 West Crossett, AR 71635 Ph. (870) 364-3167

Little knowledge or water well documentation (e.g. well logs, depth, formation, aquifer, or other construction data) is available at the refuge. Prospective bidders are encouraged to visit the sites, inspect existing conditions, inspect the wells, review performance requirements, make all necessary measurements, and perform exploratory investigation as may be required to develop accurate bid proposals. The Government shall not be held responsible for claims or extra costs associated with inaccuracies identified in description, diameter, or depth of the wells as identified in this Statement of Work.

One well is located near the Felsenthal NWR Office and Visitor Center and the other, within the Felsenthal NWR Maintenance Complex. The depth of each is unknown. Both have a steel inner casing of approximately 7 ½-inch and a outer steel casing of approximately 12 ¾-inch. The address for both locations are as follows:

Felsenthal NWR 5531 Highway 82 West Crossett, AR 71635

The contractor is responsible for site preparation (e.g. removal of trees, vegetation, equipment, and/or material) that may obstruct access to the wells prior to contract implementation. The contractor must propose, and receive approval from the refuge concerning any required site preparation associated with this project, and as a condition of this contract.

Safety, Health and Environmental Compliance In addition to the requirements of this project, the contractor shall be responsible for meeting all applicable Federal, State and local laws, regulations, codes and similar requirements relating to safety, personal protection, environmental, and transportation compliance.

Notifications, Fees, and Permits

The contractor shall identify and make all applicable regulatory notifications, obtain necessary permits, and pay all applicable fees in conjunction with this work.

Job Conditions and Work The contractor must coordinate with the refuge concerning the proposed work schedule and need of any staging area that may be required for the project. Refuge office hours are Monday-Friday 7:00 A.M-3:30 P.M, and closed on all Federal Holidays. The contractor must provide and maintain temporary barriers and signs as necessary to protect the safety of contractor employees, refuge staff, and the public.

Active and Inactive Utilities: Prior to project implementation, the contractor shall coordinate with the proper/responsible utility companies to locate and protect all underground and overhead utilities in and around the work area. The contractor is responsible for the disconnection and permanent termination of all electrical and water lines associated with each abandoned well. All work performed shall be in accordance with state and national building, plumbing, and electrical codes and standards of each governing the utility.

As previously stated, all work must be performed in accordance with the most current regulations associated with proper well abandonment and identified by the Arkansas Water Well Construction Commission (Reference Attached). Additionally, the contractor shall finish each well by backfilling and leveling the soil material around the sealed well, gently sloping the finished grade away from well, providing positive surface drainage to prevent any ponding of water. The contractor must also use concrete to patch, restore, or repair abandoned well-heads protruding from existing concrete slabs or structures, protect existing structures, utilities, roads, trails, etc. and repair any ruts, and damages caused by the contractor in executing the work.

The contractor is responsible for removing and properly salvaging, recycling, and/or disposing of all materials, equipment, and debris off-site and when the project is completed. In addition, the contractor must leave the entire work area in clean condition and free of all trash/debris, remove equipment, temporary signs and barricades.

Well Abandonment Forms and Reports The contractor shall, within 30 days of completion, submit all required documents, forms, etc. to the Arkansas Water Well Construction Commission and the refuge.
