52.212-2 ADDENDUM

EVALUATION PROCESS:

Award will be made using the Lowest Price Technically Acceptable (LTPA) source selection process to the responsible offeror whose proposal meets or exceeds the acceptability standards. The Government will initially list proposals from lowest to highest price. The Government will evaluate the lowest priced proposal first. If that proposal is determined technically unacceptable, the Government will evaluate the next lowest priced proposal and continue in that manner until a proposal is rated Technically Acceptable. If a proposal is rated Technically Unacceptable, the Government will not evaluate any other factor. USING THIS PROCEDURE, THE

GOVERNMENT MIGHT NOT EVALUATE ALL PROPOSALS.

Award may be made without discussions with offerors (except communications conducted for the purpose of minor clarification). Therefore, each initial offer should contain the offeror's best terms from a technical and price standpoint. However, the Government reserves the right to conduct discussions if it is later determined by the Contracting Officer to be necessary.

The Government will award a Firm-Fixed-Price (FFP) contract resulting from this solicitation, to the responsible quotation conforming to the solicitation, that is the lowest price technically acceptable. Award will be on an all or none basis.

A written notice of award or acceptance of an offer will be furnished via e-mail to the successful offeror within the time for acceptance specified in the offer and shall result in a binding contract without further action by either party.

Before the offer's specified expiration time, the Government may accept an offer (or part of an offer) unless a written notice of withdrawal is received before award.

The following evaluation factors shall be used to evaluate offers:

Factor 1: Technical Acceptability To receive consideration for award, a rating of no less than “Acceptable” must be achieved in technical acceptability.

ACCEPTABLE: Offer will be found technically acceptable if the Offeror’s proposal must meet or exceed all of the technical requirements set forth in the solicitation and must adhere to all of the salient characteristics outlined in the Statement of Work.

UNACCEPTABLE: An unacceptable rating will be assessed on any Offeror that demonstrates any of the following:

(i) Failure to meet any of the technical requirements (including the salient characteristics) set forth in the solicitation

(ii) Failure to provide sufficient detail for the Government to determine whether the items satisfactorily meet the requirements of this solicitation.

(iii) Failure to provide written confirmation that the mobile robots will be installed and fully functional at BAMC no later than eight (8) months following issuance of purchase order.

Minimum Clinical Performance Requirements:

Low profile to fit under Pedigo AGV Carts (measurements under cart 14"H x 24"W x

55" L) Safety Features: Alarms, notifies users if robot cannot move, Crash Alarm, emergency stop feature, proximity alert Operator Control Features: User Interfaces at each pick up and drop off location Information display features visibility of robot's location, status, and alarms Software/Hardware requirements must be a Turn Key system, access required to maintenance and user functions. BAMC will provide computer station and server.

Interoperability/connectivity with elevator system Wireless communication capabilities with user and maintenance interfaces Wireless User Interface Made for indoor use Must handle up to 1,000lbs of load capacity Requires call stations for users to signal the robots to pick up at a desired location Omni-directional movement capability

Source Considerations:

Power source: Battery Operated, rechargeable with charging stations Voltage requirements: dual, single, 110V/60Hz, 220V/50Hz for charging station

Biomedical Tech Considerations:

Not a medical device Training required for 4x Facilities Personnel

Certifications:

Requires Authorization To Operate/Risk Management Framework for use on DHA Network

Special Considerations:

Must work with existing AGV tunnel Must work with existing surgical instrument carts Pedigo AGV Carts Must communicate with elevators Must communicate with door access Have call stations within SPD Clean and Soiled side, and 2nd/4th Floor level drop offs User login security for maintenance technicians

Delivery and Installation:

The Offeror MUST specify, in writing with their offer, the Offeror’s best and final delivery terms to include the Offeror’s proposed date that the AGV/AMR system will be installed and fully functioning at Brooke Army Medical Center, which shall not exceed eight (8) months after issuance of the purchase order.

Factor 2: Price Quotes will be reviewed to determine fairness and reasonability of pricing, and an award will be made on a Lowest Price Technically Acceptable (LPTA) basis. Offeror's pricing must be determined to be fair and reasonable in order to be eligible for an award. Quotes must be submitted on time to the Contract Specialist, Jaclyn Evans, via email at jaclyn.v.evans.civ@health.mil with a courtesy copy to the Contracting Officer, Mr. Derrick Johnson, at derrick.j.johnson2.civ@health.mil. Potential contractors must be registered in the System for Award Management (SAM) to be eligible for award; the SAM website is http://www.sam.gov; a responsibility determination will be made prior to award in accordance with FAR 9.104-1.
