About this file
This is a Bid Cover Sheet for an Invitation to Bid (ITB #S-73000-00002475) issued by the Oregon Department of Transportation for the procurement of highway-grade plywood products. The ITB seeks bids for two specific items: HDO BB Sign-Grade Plywood in dimensions of 3/4" x 4' x 8' and 3/4" x 4' x 10'. The document establishes the framework for bidder submission, including requirements for bidder information, experience qualifications, certifications, and references. Bidders must provide three references for comparable projects in size and scope, including contact information for reference firms and descriptions of previously delivered goods or services. The cover sheet requires bidders to certify that they have reviewed all addenda to the ITB and agree to comply with all requirements and specifications outlined in the solicitation and sample agreement.
The bid cover sheet incorporates the Buy Oregon Preference provision under ORS 279A.128, allowing bidders to certify if the plywood items qualify for preference consideration when goods are fabricated or processed entirely in Oregon. Bidders must make multiple certifications addressing non-discrimination in employment practices, equal employment opportunity policies, absence of debarment or suspension from federal or state agencies, compliance with tax laws, and conflict of interest disclosures. For contracts anticipated to exceed $150,000, bidders must certify possession of a written policy and practice for preventing sexual harassment, sexual assault, and discrimination. The cover sheet requires the authorized representative to sign and approve the bid submission, certifying that all contents are truthful and accurate and that the bidder is an independent contractor as defined under applicable state and federal guidelines.
