About this file

The document is a Contractor Certification form from the City of New Bedford, issued under Mayor Jonathan F. Mitchell's administration. This certification requires contractors to affirm that they will not knowingly employ undocumented workers, will verify the immigration status of workers assigned to city contracts without discriminatory practices, and will not accept or create falsified worker documentation. The certification applies to all city contracts and includes potential sanctions for non-compliance, such as monetary penalties, payment withholding, contract suspension, or termination.

The certification form is specifically related to the Wastewater Treatment Plant (WWTP) Improvements project, which involves Alkalinity Addition and Instrumentation and Controls contracts. The form provides space for a contractor's authorized representative to sign, print their name, provide contact details including title, telephone, fax, and email, and date the document. By signing, the contractor agrees to comply with federal and state immigration requirements and understands the potential consequences of breaching these terms during the contract period.
