About this file
This document is an OSHA Certification Requirement form for the City of New Bedford's Howard Avenue Pumping Station Improvements project, applicable to contractors and subcontractors working on public building and public works sites. Effective since July 1, 2006, the form mandates that all employees working on the project must have successfully completed a minimum 10-hour construction safety and health course approved by the United States Occupational Safety and Health Administration (OSHA) prior to beginning work. Contractors are required to submit OSHA completion cards for each employee to the City of New Bedford before project commencement and must continue to supplement these documents as new employees are hired or contracted.
The form serves as a certification document that requires the authorized representative of a joint venture, corporation, partnership, or other legal entity to verify compliance with OSHA training requirements for employees working on the project. It specifically references Massachusetts General Laws Chapter 30 §39M and Chapter 149, indicating this is a regulated public works project with strict safety training prerequisites. The document provides space for the representative to print their name, position, company affiliation, and date of signing, creating a formal commitment to maintaining proper safety training standards for all workers involved in the Howard Avenue Pumping Station Improvements project.
