About this file
This document is an OSHA Certification Requirement form for the City of New Bedford, Massachusetts, related to a Wastewater Treatment Plant (WWTP) Improvements project. The form mandates that all contractor employees working on public building and public works sites must have successfully completed a minimum 10-hour OSHA-approved construction safety and health course, effective July 1, 2006. The certification requires bidders to confirm that all employees will have completed this safety training before beginning work on the project.
The form requires contractors to submit OSHA completion cards for each employee to the City of New Bedford prior to commencing work, with ongoing supplementation as new employees are hired or contracted. The document specifically references Massachusetts General Laws (Chapter 30, §39M and Chapter 149) and is part of the bidding requirements for a public works project involving improvements to the Wastewater Treatment Plant, which includes sub-bid opportunities in areas such as masonry, electrical work, plumbing, HVAC, and other specialized construction trades.
