About this award

The Health Resources and Services Administration awarded the Department of Public Health Illinois $4,752,673 on April 1, 2026, under the HIV Care Formula Grants program (CFDA 93.917) to provide AIDS Drug Assistance Program emergency relief funding addressing anticipated shortfalls from January 2026 increases to marketplace and commercial health plan expenditures and late-2026 Medicaid cuts.

Illinois ADAP will deploy the funds to purchase antiretroviral medications and insurance premiums for eligible clients, targeting program sustainability and preventing formulary restrictions, income eligibility reductions, and waitlist creation. The hybrid medication assistance program served over 10,900 clients with medication assistance and 2,877 with premium assistance in FY25, achieving a viral suppression rate of approximately 92 percent among served clients. The program seeks to maintain viral suppression and service delivery for current and new clients while implementing cost-saving strategies aligned with HRSA standards.

Performance runs April 1, 2026 through March 31, 2027, with work performed in Illinois.

Description

Adap shortfall relief - illinois adap (ILA) is applying for the competitive adap emergency relief funding (ERF) opportunity through the health resources and services administration (HRSA)- 26 -012 in anticipation of significant changes to health care (i.e. january 2026 increase to marketplace and other commercial health plan expenditures; late 2026 medicaid cuts and increased assessment requirements). Combined with the program's continual growth, these factors present cause for concern and will place considerable strain on the ILA program, potentially leading to formulary restrictions and reductions in client income eligibility, and an adap waitlist. To this end, ILA, if awarded the proposed $7 million in FY26 ERF funds, will directly target program sustainability by purchasing medication and insurance premiums for eligible clients. ILA is considered a hybrid medication assistance program (serving those insured and uninsured). Since FY23, ILA has been stably operating the program while maximizing the quality and quantity of delivered service. In FY25, the program has served over 10,900 clients with medication assistance and 2,877 with premium assistance. The viral suppression rate among these clients is approximately 92%. Overall increased expenditures for premium assistance and FY26 cuts to medicaid, however, threaten the program's ability to maintain viral suppression and optimal health among illinoisians living with HIV. ILA has thoughtfully planned and forecasted for anticipated challenges and has developed an approach to maintaining service delivery through strategic initiatives and partnerships. ILA needs ERF funding to support current and new clients and will implement this grant in alignment with its commitment to HRSA standards if awarded. Combined with cost-saving strategies, an ERF award to ILA would help the program delay or prevent service reductions. IT may ultimately protect clients from an adap waitlist. This critical work is not only vital to the programmatic health of the ILA but also to the health of all clients served.
