Description

Abstract
Atopic dermatitis (ad) is a chronic skin condition that significantly diminishes quality of life due to persistent itching and inflammation. The dysfunction of the skin barrier, largely attributed to the overactivity of key proteases, is a crucial factor in ad pathogenesis. Current therapeutic options, primarily based on immunomodulation, are expensive and carry notable risks. Moreover, they fail to correct the fundamental barrier defect, necessitating indefinite treatment and exposing patients to potential long-term side eeects. In response to this challenge, we have devised an innovative treatment approach utilizing a non-pathogenic bacterium, engineered to deliver a trio of targeted protease inhibitors directly to the compromised skin barrier. This method is orthogonal and complementary to current immunotherapy and could potentially transform long-term ad management. During our phase i sbir eeorts, we demonstrated the potential for our therapy to deliver and sustain a novel protease inhibitor with high eeicacy across the epidermis in both EX vivo and in vivo models. For phase II, we extend this foundation into a comprehensive multi-protease inhibitor-producing cell therapy. We plan to demonstrate its eeicacy in an atopic murine model specifically designed to mimic protease-driven barrier damage, propelling US toward our pre-IND meeting with the FDA. This proposal leverages the unique ability of our platform for continuous, localized production of therapeutic proteins, promising a groundbreaking advancement in ad treatment. If successful, this could significantly reduce the economic burden of ad therapies, providing a safer, more eeective, and cost-eeicient treatment alternative for millions aeected by this debilitating condition.
