TYPE 1 TOGETHER combines peer mentoring from trained peer mentor families with community health worker support to address personal, behavioral, and environmental barriers to consistent CGM use. The intervention targets adolescents whose suboptimal adherence to CGM—wearing it fewer than 80 percent of the time—increases risk for acute and long-term diabetes complications. The trial will recruit 180 adolescent-caregiver dyads across three sites: Nemours Delaware (Wilmington, Delaware, the place of performance), Nemours Orlando, and University of Tennessee Memphis. Participants will be randomized 1:1 to receive TYPE 1 TOGETHER or a six-month waitlist control, with families meeting at least monthly with a peer mentor family and community health worker during the intervention phase.
