About this award
The Department of Health and Human Services Centers for Disease Control and Prevention awarded $125,000 to County of Sublette under the Drug-Free Communities Support Program (CFDA 93.276) on July 1, 2026.
The Sublette County Prevention Coalition will implement a comprehensive, community-driven prevention initiative to reduce youth alcohol, nicotine, and opioid misuse in Sublette County, Wyoming (ZIP codes 82941, 83113, 83115, 83122, 82925). The initiative addresses documented local need: 2024 Prevention Needs Assessment data show 21.08% of Sublette County high school students reported past 30-day alcohol use (above Wyoming's 19.64% average), nicotine vaping is more frequent than marijuana vaping among high school students, and 6% of 12th graders reported lifetime prescription pain medication misuse. Geographic isolation in this rural frontier environment limits structured youth activities and increases exposure to adult substance use in common recreation settings.
The coalition will deploy the DFC Seven Strategies for Community-Level Change using evidence-based frameworks including Communities That Care and the Strategic Prevention Framework. Activities include data-informed public education and media campaigns to increase youth perception of risk and parental disapproval, evidence-based skill-building for youth and adult influencers, expanded youth-led substance-free activities and mentoring to reduce unstructured time, and improved connections to prevention-focused early intervention services.
The period of performance runs from July 1, 2026 through June 30, 2031, with place of performance in Wyoming.
Description
CE26-0220 - sublette county drug-free communities (DFC) support program - new (year 1) recompete - the sublette county prevention coalition (SCPC) will implement a comprehensive, community-driven prevention initiative to reduce youth alcohol, nicotine, and opioid misuse in sublette county, wyoming (ZIP codes 82941, 83113, 83115, 83122, 82925), aligned with the two statutory goals of the drug-free communities (DFC) support program: (1) strengthen collaboration among community stakeholders and (2) reduce youth substance use by addressing community risk factors and increasing protective factors. Local data demonstrate the need for focused action. The 2024 prevention needs assessment (PNA) shows 21.08% of sublette county high school students reported past 30-day alcohol use (above the wyoming average of 19.64%). Nicotine vaping is a persistent concern, with nicotine vaping reported more frequently than marijuana vaping among high school students. The 2024 PNA also indicates 6% of sublette county 12TH graders reported lifetime misuse of prescription pain medication. These patterns occur in a rural frontier environment where geographic isolation limits structured, supervised youth activities and increases exposure to adult-modeled substance use in common recreation settings. Additional local partners (schools, DFS, and healthcare stakeholders) confirm earlier initiation trends and ongoing family impacts. SCPC will carry out a comprehensive prevention strategy using the DFC seven strategies for community-level change and evidence-based frameworks (communities that care and the strategic prevention framework). Activities will include (1) data-informed public education and media campaigns to increase youth perception of risk and parental disapproval, (2) evidence-based skill-building for youth and adult influencers, (3) expanded youth-led, substance-free activities and mentoring to reduce unstructured time, (4) improved connections to prevention-focused early intervention supports, (5) incentives and alternative consequences that reinforce healthy norms, (6) environmental improvements at community gatherings to reduce youth access and exposure, and (7) education and technical assistance to local decision-makers on policies and practices that reduce youth access and availability of alcohol and nicotine products. SCPC will also strengthen coalition capacity and sustainability by increasing sector engagement and youth leadership. A youth council with defined roles and voting participation will help plan and implement prevention activities and contribute to strategy prioritization. Evaluation will align with DFC national cross-site evaluation requirements using biennial PNA data (past 30-day use, perception of risk, parental disapproval, and peer disapproval) and quarterly performance monitoring via a coalition dashboard and scorecard to support continuous improvement. By the end of the 12-month action plan period (2026-2027), SCPC expects to increase coalition participation and effectiveness, strengthen youth leadership, improve perception of risk and disapproval norms, and make measurable progress toward reductions in youth alcohol use, nicotine use, and prescription opioid misuse, supporting long-term sustainable community-level change.
