THIS GRANT PROVIDES $43392 OF FFY 2020 OPERATING ASSISTANCE TO THE NON-PROFIT SUBRECIPIENT MISSING PIECES CDC FOR THE PERIOD OF APRIL 1 2022 THROUGH AUGUST 1 2023. THE LOCAL SHARE FOR OPERATING IN THE AMOUNT OF $43392 WILL BE PROVIDED BY MISSING PIECES CDC. PURPOSE: THE PURPOSE OF THIS APPLICATION IS TO PROVIDE 1+ YEARS OF OPERATING ASSISTANCE FOR MISSING PIECES CDC TO CREATE A NEW SERVICE THAT AIMS TO INCREASE OPTIONS FOR SENIORS AND INDIVIDUALS WITH A DISABILITY TO GET TO MEDICAL APPOINTMENTS.ACTIVITIES TO BE PERFORMED: FUNDS WILL BE USED TO COVER ALL ELIGIBLE OPERATING EXPENSES SUCH AS DRIVER SALARIES FUEL INSURANCE AND OTHER ITEMS HAVING A USEFUL LIFE OF LESS THAN ONE YEAR.EXPECTED OUTCOMES: THE EXPECTED OUTCOME IS A DECREASE IN THE NUMBER OF MISSED APPOINTMENTS AND GETTING THOSE MOST IN NEED TO THEIR MEDICAL APPOINTMENTS. THE GOAL IS THAT THIS SERVICE RELIES LESS ON SECTION 5310 FUNDING OVER TIME DUE TO ITS SUCCESS AND OTHER ORGANIZATIONS SEEING THE NEED TO HELP FUND THIS SERVICE IN ORDER TO CONTINUE IT FOR MANY YEARS. INTENDED BENEFICIARIES: THE INTENDED BENEFICIARIES ARE SENIORS AND INDIVIDUALS WITH A DISABILITY THAT HAVE A DIFFICULTY IN GETTING TO SCHEDULED AND MUCH NEEDED MEDICAL APPOINTMENTS.
