This $1,200,000 Project Grant award, provided by the U.S. Department of Health and Human Services' Health Resources and Services Administration (HRSA) under the Health Center Program (CFDA 93.224), will establish the El Centro SUD CARES model in New Mexico. The target population includes hard-to-reach individuals with substance use disorder and opioid use disorder who are often justice-involved or experiencing homelessness. The team will provide intensive MOUD treatment, recovery support, care coordination, peer support, and linkages to social services, meeting patients in non-traditional settings like their homes, jails, and shelters. This innovative home-visiting model aims to build trusting relationships and better serve the complex needs of this underserved population that cannot be addressed in a traditional clinic setting. The award will fund 1.5 FTE MOUD providers, 1 FTE substance use counselor, 2 FTE community health workers, 1 FTE peer recovery specialist, 1 FTE health navigator, and 1 FTE administrative staff. It is expected that this model can be replicated in rural communities across New Mexico and the nation to address the most complex substance use cases.
