The $500,000 per year federal Project Grant award from the Health Resources and Services Administration's (HRSA) Community Health Centers program (CFDA 93.224) will allow Community Health Center, Incorporated (CHCI) to expand its primary care service hours at multiple locations in Connecticut. The funding will enable CHCI to offer evening and weekend in-person clinical services, as well as expanded virtual telehealth care on Sundays, to better meet the needs of its underserved and vulnerable patient population. This includes low-income, uninsured, and special populations such as migrant workers, immigrants/refugees, and the unhoused. The expanded hours are expected to increase CHCI's capacity to accept new patients, improve access to primary care, and reduce unnecessary emergency and urgent care utilization. CHCI has over 50 years of experience providing comprehensive primary care, dental, behavioral health, and specialty services to over 150,000 patients across Connecticut.
