Description
- Abstract
- Summary in response to hrsa-22-154, ampla health, the largest hrsa-funded federally qualified health center (fqhc) in inland northern california has convened a regional collaborative comprised of the UC davis comprehensive cancer center/UC davis cancer care network (ucdccc), adventist rideout hospital, and county public health departments in its largely rural service area, the size of new jersey. This collaborative's vision is: "to save lives through substantially increasing colorectal cancer screening by deployment of customized, evidence-based interventions". Founded on a memorandum of understanding signed with ucdccc two years ago and renewed in 2022, this collaborative's objectives will substantially increase the percentage of ampla health patients, ages 50-75 from 35.1% to 50.0% to meet the US preventive services task force grade a recommendations for colorectal cancer screening; linking patients with abnormal screening results to appropriate follow-up within 30 days; enhancing ampla health's capacity for evidence-based interventions that utilize in-person approaches for clinic visits and outreach to patients including migrant farm workers; and evaluating progress. For the more than 90% of ampla health patients with smartphones, we will deliver video messages in four languages that will cover more than 96% of patients' primary languages. To address the lack of household vehicles in approximately 6% of households, we will furnish ampla health transportation. Our county health departments will reinforce the importance of colorectal cancer screening. This implementation plan is customized based on ampla health's distinctive patient demographics. Compared to the US at-large, ampla health's patients reflect a majority of racial/ethnic minorities (52.7%), proportionally more rural (44%>14%); and much higher poverty rates (table 2). Four of the six ampla health counties experience higher colorectal cancer mortality rate s than california and the US. Built into the implementation are evaluative measures that will be monitored by monthly meetings of key personnel from ampla health and ucdccc as well as semi-annual meetings of the collaborative partners. Milestones toward attainment of objectives include a detailed work plan, a logic model, and a detailed task/timeline.
