FY 2021 ENDING THE HIV EPIDEMIC - PRIMARY CARE HIV PREVENTION - GREATER FAMILY HEALTH WILL USE GRANT FUNDS TO EXPAND HIV PREVENTION SERVICES AND DECREASE THE RISK OF HIV TRANSMISSION IN SUBURBAN COOK COUNTY, IL, 1 OF 57 PRIORITY COUNTIES PART OF PHASE 1 OF THE ENDING THE HIV EPIDEMIC (EHE) FEDERAL RESPONSE. FUNDING WILL ALSO EXPAND HIV PREVENTION SERVICES IN THE NEARBY COUNTIES OF DEKALB, DUPAGE, KANE, AND MCHENRY. THE PROPOSED PROJECT WILL ACHIEVE ALL THREE OBJECTIVES OF THE EHE PRIMARY CARE HIV PREVENTION (PCHP) PROGRAM BY IMPLEMENTING THE FOLLOWING ACTIVITIES: 1) INCREASE THE NUMBER OF PATIENTS COUNSELED AND TESTED FOR HIV - GREATER FAMILY HEALTH WILL USE FUNDING TO ENHANCE WORKFLOWS TO SUPPORT UNIVERSAL HIV TESTING AND ENHANCE ITS ELECTRONIC HEALTH RECORD SYSTEM WITH CLINICAL DECISION SUPPORT TOOLS. 2) INCREASE THE NUMBER OF PATIENTS PRESCRIBED PREP - GREATER FAMILY HEALTH WILL USE FUNDING TO SUPPORT PREP ACCESS AND ADHERENCE BY HIRING A NEW CARE MANAGER TO SERVE AS A PREP NAVIGATOR RESOURCE FOR PATIENTS AND PRACTITIONERS. THE PREP NAVIGATOR WILL BE A SUBJECT MATTER EXPERT IN ALL PREP-RELATED MATTERS, INCLUDING ACCESSING MEDICATION ASSISTANCE AND DONATION PROGRAMS, COUNSELING PATIENTS AND PRACTITIONERS ON AVAILABLE OPTIONS, AND FOLLOWING-UP WITH PATIENTS TO ENSURE MEDICATION ADHERENCE. GREATER FAMILY HEALTH WILL ALSO USE FUNDING TO REVISE POLICIES AND PROCEDURES TO INCREASE ORGANIZATIONAL COMPETENCY ON THE EXPANDED HIV PREVENTION SERVICES. 3) INCREASE THE NUMBER OF PATIENTS LINKED TO HIV CARE AND TREATMENT WITHIN 30 DAYS OF DIAGNOSIS - GREATER FAMILY HEALTH WILL USE FUNDING TO BUILD NEW AND ENHANCE EXISTING CARE COORDINATION INFRASTRUCTURE TO ENSURE THAT ALL NEWLY DIAGNOSED HIV PATIENTS ARE LINKED TO AND ENGAGED IN TREATMENT WITHIN 30 DAYS OF DIAGNOSIS. GREATER FAMILY HEALTH WILL ALSO USE FUNDING FOR ADDITIONAL WORKFORCE DEVELOPMENT AND OUTREACH ACTIVITIES THAT FURTHER FACILITATE LINKAGE AND TREATMENT WITHIN 30 DAYS OF DIAGNOSIS. GREATER FAMILY HEALTH WILL FOCUS ON THE TARGETED GEOGRAPHIC LOCATI ON (COOK COUNTY) BY IMPLEMENTING THE ABOVE ACTIVITIES AT ITS TWO HEALTH CENTERS LOCATED IN COOK: 1) 135 E IRVING PARK RD, STREAMWOOD, IL 60107; AND 2) 300 MCHENRY RD, WHEELING, IL 60090. GREATER FAMILY HEALTH WILL ALSO EXPAND HIV PREVENTION SERVICES IN THE NEARBY COUNTIES (DEKALB, DUPAGE, KANE, AND MCHENRY) BY IMPLEMENTING THE ABOVE ACTIVITIES AT SIX ADDITIONAL HEALTH CENTERS THAT IT OPERATES, COMPRISING THE ENTIRETY OF ITS BUREAU OF PRIMARY CARE-APPROVED SERVICE AREA.
