Description
Health center program - laguna beach community clinic (LBCC) is a federally qualified health center look alike providing comprehensive and integrated primary medical, preventative dental, vision, and behavioral health services to the low-income residents of southwest orange county, california. LBCC's target population is those living at or below 200% federal poverty level, in laguna beach and surrounding communities of irvine, capistrano beach, costa mesa, dana point, lake forest, laguna woods, laguna hills, aliso viejo, newport coast, newport beach, SAN clemente, SAN juan capistrano, laguna nigel, rancho santa margarita, mission viejo, ladera ranch, and santa ANA. LBCC has been serving the community since 1970 when IT opened its doors as a free clinic, and became a licensed community clinic in 1985, adopting the name "laguna beach community clinic". LBCC applied for and received ryan white funding in 1990 to expand their treatment of patients with HIV. In 1994, the clinic purchased the current building with generous support from private grant funding and community support. LBCC received fqhc look alike status in december 2018 and is operating in full compliance with all health center program requirements. The service area of this proposal has a mere 30.24% health center penetration of low-income population. There are 164,334 low-income individuals in the service area who are not currently being served by a health center. This represents a dire need for increased access to affordable health care for the low income underserved population. This project will address the need for comprehensive primary health care services in the community and service area by providing general primary medical care, obstetrics and gynecology, well child services, immunizations, behavioral health, preventative dental care, nutrition, case management, eligibility assistance, health education, outreach, transportation and translation services. Services will be provided on a sliding fee scale for the uninsured. LBCC proposes to serve 3,120 patients (9,360 visits) by the year 2026. The health center service delivery site is a two story facility with 9 exam rooms, serving the community full time, monday through saturdays. A new site will be opened at a local hospital to provide primary health care to those using the emergency room for non-urgent care. As a current fqhc look-alike, site operations are in hrsa compliance, and staff members are prepared for a successful implementation of this new access point grant project.
