The Department of Health and Human Services Health Resources and Services Administration awarded Crossroads Health $1.3 million under the Health Center Program (CFDA 93.224) on August 1, 2026, to establish a new access point and expand primary care and behavioral health services to an underserved population in northeast Ohio. Crossroads Health will use the funding to serve approximately 1,000 new patients through two service delivery locations in Mentor and Cleveland, Ohio. The recipient provides primary care, pharmacy, psychiatry and medication management, substance use disorder treatment including dual diagnosis services, school-based services, early childhood services, outpatient mental health, intensive outpatient services, transitional housing, and criminal justice services. The service area encompasses all of Lake County and the northeastern portion of Cuyahoga County, where more than 114,000 low-income individuals reside; nearly 98,000 remain unserved by the Health Center Program. Both counties experience elevated mortality rates for leading causes of death, including unintentional drug overdose at 44.2 per 100,000 in Cuyahoga County and 38.9 per 100,000 in Lake County, compared to 32.6 nationally. Many individuals in the target population have co-occurring mental health and substance use disorder diagnoses. The award runs through July 31, 2027, with place of performance in Mentor, Ohio.
