The initiative targets adults 18 and older experiencing barriers to sustained recovery, including unstable housing, unemployment, transportation challenges, limited recovery support networks, justice system involvement, and limited access to recovery-oriented services. Services will coordinate with healthcare providers, treatment organizations, medication for opioid use disorder providers, and employers to improve recovery stability, increase employment and housing outcomes, and strengthen community recovery infrastructure.
The project will serve 175 unduplicated participants in year one, 210 in year two, and 250 in year three, for a total of 635 unduplicated participants across the three-year period of performance (September 30, 2026 through September 29, 2029). Services extend to eligible family members alongside the primary adult population. Live Rite is a woman-owned, tax-exempt nonprofit organization based in Roseville and registered with the federal government since June 2017.
Description
Housing, uplift, and belonging (hub): expanding recovery support services in macomb county - the housing, uplift, and belonging (hub) initiative strengthens long-term recovery for adults with substance use disorder (SUD) and CO-occurring disorder (COD) by expanding access to peer recovery coaching, recovery housing, workforce development, transportation assistance, family recovery coaching, and coordinated recovery support services (RSS) throughout macomb county, michigan. The initiative will improve recovery stability, increase employment and housing outcomes, reduce barriers to care, and strengthen the community's recovery infrastructure through collaboration with healthcare providers, treatment organizations, employers, and other recovery partners. Macomb county, michigan, is located in southeast michigan and is home to approximately 886,175 residents. The hub initiative will primarily serve adults 18 years of age and older residing throughout macomb county, while also providing recovery support services to eligible family members. The project will serve 175 unduplicated participants in year 1, 210 participants in year 2, and 250 participants in year 3, for a total of 635 unduplicated participants over the three-year project period. The target population includes adults with SUD and COD who are in or seeking recovery and who frequently experience barriers to sustained recovery, including unstable housing, unemployment, transportation challenges, limited recovery support networks, justice system involvement, and limited access to recovery-oriented services. Many participants will also require coordination with behavioral health providers, healthcare providers, medication for opioid use disorder (MOUD) providers, recovery housing, workforce development programs, and other community resources. The hub initiative will be coordinated through live rite's 14,000-square-foot recovery resource center in roseville and will provide evidence-informed peer recovery support services (PRSS) and recovery support services (RSS), including peer recovery coaching, case management, recovery housing navigation, transportation assistance, family recovery coaching, workforce development and employment services, recovery-ready workplace (RRW) initiatives, overdose prevention education, tobacco cessation services, support groups, and warm handoffs to treatment, healthcare, and other community partners. Services will be individualized through participant-driven recovery planning and coordinated with a multidisciplinary network of treatment providers, healthcare organizations, employers, and recovery organizations. The goals of the hub initiative are to: (1) expand access to high-quality peer recovery support services and recovery support services for adults with SUD and COD; (2) improve recovery stability by increasing access to recovery housing, transportation, employment, and family recovery supports; and (3) strengthen the local recovery infrastructure by expanding the peer recovery workforce, implementing recovery-ready workplace initiatives, and enhancing collaboration among community partners. Key measurable objectives include serving 635 unduplicated participants over three years; increasing participant engagement in recovery support services; expanding access to recovery housing and supportive services; improving employment outcomes through workforce development and industry-recognized certifications; training and mentoring new certified peer recovery coaches; strengthening employer participation in recovery-ready workplace initiatives; and improving coordination across the recovery continuum through collaborative partnerships and continuous quality improvement activities.
