Description
Treatment and recovery services for youth, young adults, and families - LA maestra family clinic, INC. (LA maestra) requests $545,000 annually for five years through the substance abuse and mental health services administration (SAMHSA) youth and young adult treatment (YTREE) program to expand evidence-based substance use disorder (SUD) and CO-occurring disorder (COD) treatment services for children, adolescents, and young adults (ages 12-25) residing in SAN diego county. As a federally qualified health center (FQHC), LA maestra provides integrated primary care, mental health services, psychiatric care, outpatient SUD treatment, recovery support services, and care coordination through the LA maestra circle of care® model. The proposed project will improve timely access to comprehensive, family-centered care by integrating screening, brief intervention, and referral to treatment (SBIRT), motivational interviewing (MI), cognitive behavioral therapy (CBT), peer recovery support services, family engagement, care coordination, telehealth, and medications for opioid, alcohol, and tobacco use disorders when clinically indicated. Participants will receive comprehensive assessments, individualized treatment and recovery plans, recovery support services, and referrals to primary care, mental health services, specialty treatment, education, housing, employment, child welfare, developmental disability services, and other community resources based on individual needs. During the five-year project period, LA maestra will enroll 50 unduplicated children, adolescents, and young adults during project year 1 and 100 unduplicated participants annually during project years 2-5, serving a total of 450 unduplicated participants. The project will ensure that 100% of participants receive a comprehensive assessment, an individualized treatment and recovery plan, and appropriate evidence-based services within 30 days of enrollment; 100% of participants will receive recovery support services, care coordination, transition planning, and referrals, as appropriate; and 80% of participants will have at least one family member or caregiver participate in treatment planning, family education, family therapy, or another family support activity when clinically appropriate. LA maestra will collaborate with children of the rainbow, turn behavioral health services, mcalister institute, rady children's hospital, the SAN diego county office of education's juvenile court and community schools (JCCS), optum, and the SAN diego regional center to strengthen care coordination and ensure participants have access to the full continuum of services. Program performance and outcomes will be monitored through nextgen, I2ITRACKS, gpra, and the SAMHSA performance accountability and reporting system (SPARS). An independent evaluator from the university of california, SAN diego health services research center will conduct ongoing program evaluation to assess progress toward project goals, support continuous quality improvement, and promote long-term sustainability of evidence-based services. By expanding access to integrated, evidence-based treatment and recovery support services, the proposed project will improve health outcomes, strengthen families, and promote long-term recovery for children, adolescents, and young adults with substance use disorders and CO-occurring disorders throughout SAN diego county.
