NARA NW will deliver substance use screening, education, brief intervention, referral to treatment, case management, and wraparound services to the American Indian and Alaska Native population in the Portland metropolitan area, including Clackamas, Multnomah, and Washington Counties. At least 50 percent of those served will be youth, with the project anticipating service to 2,760 individuals during the performance period. The organization will expand SBIRT implementation for families and young people, develop a 10-week psychoeducation curriculum addressing substance use and mental health for emerging adults, and strengthen integrated care through shared care plans for clients with positive SBIRT screenings. Early screening and intervention will begin at age 12, with timely referrals for individuals with substance use disorders. Success metrics include establishing partnerships with two community stakeholders, creating the 10-week curriculum, and identifying two to three SBIRT champions across mental health and medical teams.
