About this award

The Substance Abuse and Mental Health Services Administration (SAMHSA) awarded the North Carolina Department of Health and Human Services $750,000 on September 30, 2026, under the Center for Mental Health Services Projects of Regional and National Significance program (CFDA 93.532) to operationalize a magistrate-to-988-mobile crisis team (MCT) pathway in Judicial District 7, covering Bertie, Halifax, Hertford, and Northampton counties.

NC DHHS will formalize a connection between magistrates receiving involuntary commitment petitions and Real Crisis's 988 crisis backline, which dispatches Integrated Family Services to conduct community-based first examinations before custody orders issue. The state will enhance IFS MCT capacity toward 24/7/365 availability across all four counties and execute formal memoranda of understanding with courts, law enforcement, public service answering points, emergency medical services, and hospitals to enable cross-system data sharing and coordination. The initiative builds on the proven Avery/Watauga model, which reduced involuntary commitment rates by 60%, and operationalizes North Carolina Executive Order 33 and House Bill 1104. IFS already resolves 89.9 percent of behavioral health crises safely in the community; of 307 MCT responses in fiscal year 2025–26, only 3 originated from a magistrate referral. The four-county area represents among North Carolina's highest-poverty rural communities, with poverty rates ranging from 15.2 to 22.8 percent and no inpatient psychiatric facility in any county. Performance extends through September 29, 2030. Place of performance is Raleigh, North Carolina.

Description

NC behavioral health mobile crisis team partnerships - NC dmhddsus proposes to implement a magistrate to 988---MCT pathway in judicial district 7 (JD7), connecting an existing, proven mobile crisis team (MCT) resource to magistrate decision-making in bertie, halifax, hertford, and northampton counties before involuntary commitment (IVC) custody orders are issued. In JD7, integrated family services (IFS) already resolves 89.9% of behavioral health (BH) crises safely in the community - yet of 307 MCT responses in FY 2025-26, only 3 originated from a magistrate referral. This grant closes that integration GAP, operationalizing executive order 33 and HB 1104 and building on the proven avery/watauga model that reduced IVC rates by 60%. Samhsa mobile crisis team partnerships (MCTP) funding enables NC dmhddsus, as the NC state BH crisis system authority, to formalize the magistrate-988-MCT connection that state law and executive action have directed. Need: north carolina's IVC system is significantly overused, misused, and harmful (disability rights NC, june 2025). Statewide, 63% of IVC petitions (AVG. 95,191/year, 2019-2024) did not result in commitment, yet each triggered LE transport and ED boarding averaging 65.4 hours (NC-step, 2024). JD7'S four counties are among NC's highest-poverty, rural communities, with all four exceeding the 12.5% statewide poverty rate (halifax 22.8%, hertford 21.4%, bertie 19.8%, northampton 15.2%). No inpatient psychiatric facility exists in any of the four counties. Strategies: (1) formalize the magistrate to 988---MCT pathway: when a magistrate receives an IVC petition, the magistrate calls real crisis's 988 backline, which dispatches IFS to conduct a community-based first examination before any custody order is issued; (2) enhance IFS MCT capacity toward 24/7/365 availability in all four JD7 counties; (3) execute formal mous with courts, law enforcement, public service answering points (PSAPS), EMS, and hospitals to enable cross-system data collection and accountability; (4) deliver evidence-based training to magistrates, law enforcement, EMS, and community partners; and (5) develop a sustainability plan anchored in medicaid billing, 988 telecom fees, and LME/MCO (trillium health resources) contracts.
