Description

Tarrant county clinical high risk for psychosis (CHR-P) - the tarrant county clinical high risk for psychosis (CHR-P) program is a community-based initiative designed to identify and intervene early with youth and young adults at risk for psychosis. The program will strengthen early detection, expand access to evidence-based services, and improve outcomes through a coordinated, multidisciplinary approach. By integrating screening, outreach, and stepped-care interventions within an established behavioral health system, the project aims to reduce progression to psychosis and improve long-term functioning. The tarrant county CHR-P program will be implemented by mhmr of tarrant county, serving individuals across tarrant county, texas, including fort worth and surrounding communities. The service area includes a population of over two million residents. The program will serve 30 individuals in year 1, 60 in years 2 and 3, and 30 in year 4, for a total of 180 unduplicated participants over the project period. The population of focus includes youth and young adults ages 14 to 25 who are experiencing early warning signs associated with clinical high risk for psychosis. Participants will include individuals of all genders and socioeconomic backgrounds, with a significant proportion expected to come from low-income and underserved communities. Many participants may have involvement in multiple systems, including behavioral health, child welfare, education, or juvenile justice. Clinically, participants may present with attenuated psychotic symptoms, functional decline, social withdrawal, and CO-occurring mental health conditions such as depression, anxiety, trauma-related disorders, and substance use, particularly cannabis use. Many participants will require early intervention services to prevent progression to first episode psychosis and to address disruptions in education, employment, and social functioning. The project will implement a comprehensive set of strategies and interventions aligned with samhsa requirements, including outreach and education, training of providers, structured screening and assessment using validated tools, and a stepped-care model of service delivery. Evidence-based practices will include cognitive behavioral therapy for psychosis, supported employment and education, psychoeducation, trauma-informed care, and substance use interventions. Services will be delivered through a multidisciplinary team and in community-based settings to maximize accessibility and engagement. The primary goal of the program is to improve early identification, access to care, and functional outcomes for youth and young adults at clinical high risk for psychosis. Secondary goals include increasing screening and outreach, improving timely access and engagement in services, enhancing clinical and functional outcomes, and strengthening coordinated, person-centered care. Measurable objectives include screening at least 100 individuals annually, serving a total of 180 participants, initiating services within 14 days for at least 80% of participants, maintaining at least 75% engagement, and achieving functional improvement in at least 70% of participants. Additional objectives include increasing outreach and training activities and reporting required samhsa performance indicators, including the number of individuals trained, screened, reached through education, receiving peer support, and receiving care coordination.
