LIFEWAYS INTEGRATED HEALTH PROGRAM'S INTENDED OUTCOMES ENCOMPASS THE PROMOTION OF HEALTH EQUITY FOR PEOPLE WITH SPMI, SED, SUD AND CO-OCCURRING CHRONIC HEALTH CONDITIONS.
