Description

Collaborative homeless solutions - the city of norco proposes to use samhsa funds to address gaps in program services, increase placements in motel shelter and permanent housing, as well as connect clients to behavioral health services for those who are at-risk of or living with mental illness and substance use disorders. Program services will use an evidence-based model that provides trauma-informed care to ensure that the highest barrier clients receive the support they need to move from homelessness to stabilization in emergency shelter and permanent housing. Components of service will include outreach, engagement, case management, wrap-around supportive services, motel emergency shelter, food, transportation, and rapid rehousing. Program services will be provided through a contract with city net. City net operates in many different cities that are located in 8 different counties in southern and northern california. City net will use data collection standards that are in compliance with riverside county's homeless management information system (hmis). City net will generate required data reports from hmis. In addition, clients will be assessed using riverside county's coordinated entry system VI-spdat survey (vulnerability index service prioritization decision assistance tool). Client's that meet HUD's definition for chronically homeless will be referred for placement in permanent supportive housing (PSH). If PSH units are not immediately available due to wait lists, city net will use samhsa funds to provide housing assistance that will be combined with case management and ongoing services to facilitate housing retention. Transitional homeless will receive rental assistance and case management services while they are concurrently placed on the county's waitlist for section 8 rental assistance and/or other affordable housing programs. The city net contract will be managed by the city of corona homeless solutions manager through an MOU between both cities.
