The Wellstar Health System was awarded a $5,500,000 Healthy Start Initiative-Eliminating Racial/Ethnic Disparities project grant (CFDA 93.926) from the Health Resources and Services Administration (HRSA). The grant will fund the implementation of the Community-based Ecosystem of Collaborative Maternal Care (CMC) Healthy Start program to reduce infant mortality and decrease disparities in infant mortality and perinatal health outcomes among Black/African American mothers in Troup, Butts, and Spalding counties in West Central Georgia.

The CMC program will provide: (i) comprehensive individualized care coordination through an innovative staffing model embedding health equity-focused case managers; (ii) community-based, two-generation, wraparound support services to address social determinants of health; and (iii) cohort-based, culturally competent health education and parenting classes. The program aims to serve 700 participants annually and is scheduled to run from May 1, 2024 to March 31, 2029.
