Description
- Abstract
- Heart failure (HF) is the primary cause hospitalization for patients 65 years or older in the united states, and a leading cause of death worldwide. There remain limited universal treatments for HF patients, and the 5-year survival rate is only ~50% despite various therapeutic options. The 2022 update to the guideline directed medical therapy (GDMT) for patients with HF added sodium glucose cotransporter- 2 inhibitors (SGLT2I) as a core treatment option. Although SGLT2I were initially developed for treatment of type 2 diabetes mellitus (T2DM), they are highly effective at reducing overall death and rehospitalizations for HF patients, regardless of their T2DM status. Yet, our lab and others have recently found that the beneficial effects on SGLT2I on HF are independent of SGLT2 inhibition. Additionally, the lab further characterized a potential mechanism for the beneficial effects of SGLT2I against HF by activating the synthesis of coenzyme a (COA) from pantothenate, more commonly known as vitamin B5, through activation of the pantothenate kinase (PANK) family of enzymes. Yet, a unique member of the PANK enzyme family, PANK4, impedes COA biosynthesis and may be a more promising therapeutic target. This project will interrogate the role of PANK4 in COA synthesis in the heart, and (1) provide deep insights into the regulation and biosynthesis of COA in the heart in vivo, and (2) identify the potential of PANK4 as a new therapeutic target for heart failure.
