Description
- Abstract
- Maternal morbidity and mortality are increasing in the united states, with the vast majority of deaths being preventable. These poor outcomes are not distributed evenly across the US; in the midwest and south, perinatal outcomes are significantly worse compared to the west coast. One largely unexplored link between poor maternal outcomes and geographic distribution is obstetric processes of care, namely cesarean delivery. Cesarean delivery is a key antecedent to maternal morbidity and mortality, and its use varies widely between geographic areas. The purpose of this nrsa fellowship is to utilize a measure of risk-adjusted cesarean delivery use, the cesarean delivery appropriateness score, to examine whether cesarean over- or underuse influences the occurrence of severe maternal morbidity (SMM) events, and if community poverty contributes to use of cesarean. We will leverage a massive, nationally representative dataset (epic cosmos), geographic information system (GIS) mapping, spatial analysis, and multi-level modeling to achieve these goals. We theorize that maternal risk-adjusted use of cesarean delivery and SMM events will be distributed unevenly across the country, and that community-level poverty will influence occurrence of these outcomes. This project also includes a comprehensive training plan designed to prepare an obstetric nurse to be an independent research scientist with expertise in perinatal research, GIS mapping, and the influence of system-level factors on perinatal processes and outcomes.
