About this award

The Department of the Interior Insular Affairs awarded the Government of the Commonwealth of the Northern Mariana Islands $226,465.44 on February 1, 2026, under the Capital Improvement Program (CFDA 15.083) to procure mechanical respiratory ventilators for the Commonwealth Healthcare Corporation's intensive care unit.

The award funds the purchase of additional ventilators to improve access to quality health care in underserved CNMI communities. The territory's residents experience high rates of comorbid chronic diseases including hypertension, diabetes, asthma, and obesity, driving acute respiratory failure as a frequent cause of ICU admission. Geographic isolation limits options for aero-medical evacuation, particularly during inclement weather or when acute respiratory failure complicates transport, making local ICU capacity critical. Centers for Medicare and Medicaid Services conditions of participation also mandate adequate ventilator equipment. The ventilators will enhance treatment capacity during natural and man-made disasters and support continued hospital operations in a vulnerable location distant from specialty medical facilities.

The recipient is the Government of the Commonwealth of the Northern Mariana Islands, through its Office of Management and Budget Division. Place of performance is the Northern Mariana Islands. The period of performance extends from February 1, 2026, through January 31, 2031.

Description

The purpose of this proposal under the office of insular affairs (OIA) capital improvement project (CIP) program is to improve access to quality health care in the underserved communities of the cnmi by procuring additional mechanical respiratory ventilators needed for the hospital s intensive care unit (ICU). The cnmi suffers from high rates of chronic diseases, with many individuals typically having several comorbidities. This can include conditions such as hypertension, diabetes, asthma, and obesity all coexisting at the same time. Given both the social and individual risk factors faced by many of the island residents, the long-term managing of multiple illnesses simultaneously can be extremely difficult, frequently resulting in acute respiratory failure. While an indirect effect of such diseases, acute respiratory failure requiring assisted mechanical ventilation is one of the common reasons for admission to the ICU here in the cnmi, highlighting the necessity for respiratory ventilators. Given the cnmi s geographical isolation, the growing number of ICU admissions and the severity of illnesses places the cnmi in a vulnerable location, especially when aero-medical evacuation (medevac) is needed for life saving treatments at distant medical facilities. However, this can be difficult and dangerous during inclement weather, and depending on the medical condition, such as with acute respiratory failure, this can be extremely challenging with the reduction in oxygen partial pressure despite having pressurized cabins. Thus, the cnmi s far distance from specialty medical facilities highlights the need for chcc to equip itself with an adequate amount of medical equipment to not only care for its residents who are admitted, but also to ensure acceptable treatment capacity during times of natural or man-made disasters. The necessity of respiratory ventilators has also been mandated by the centers for medicare and medicaid services (CMS) conditions of participations (COP). Project goals objectives 1:improve patient safety by using advanced medical devices in a clean and safe environment. - enhanced quality of patient services.2:optimize chcc operations, accessibility and delivery of patient care. - improved standard of care and wellbeing for all those who access chcc. 3:optimize patient care and safety when on mechanical ventilation. - minimize the adverse effects of patient-ventilator asynchrony.
