SNAHC IS A 501(C)3 NON-PROFIT URBAN INDIAN ORGANIZATION (UIO) AND FEDERALLY QUALIFIED HEALTH CENTER (FQHC) ACCREDITED THROUGH THE ACCREDITATION ASSOCIATION OF AMBULATORY HEALTH CARE (AAAHC) AND CERTIFIED AS A PATIENT CENTERED MEDICAL HOME (PCMH). SNAHC PROVIDES A FULL RANGE OF MEDICAL, DENTAL, AND BEHAVIORAL HEALTH SERVICES UNDER ONE HEALTH HOME. SNAHC'S CURRENT SERVICE AREA INCLUDES 19 ZIP CODES WITH A TOTAL POPULATION OF 575,882 RESIDENTS AND 38,000 WHO ARE AMERICAN INDIAN OR ALASKA NATIVE (AI/AN) IN SACRAMENTO COUNTY. OUR MIDTOWN LOCATION SERVES OVER 12,000 PATIENTS PER YEAR, APPROXIMATELY 30% OF WHOM ARE AI/AN. WITH THE OPENING OF OUR NEW SITE, WE ANTICIPATE TO DOUBLE OUR PATIENT POPULATION TO PROVIDE ACCESS TO OVER 24,000 INDIVIDUALS PER YEAR.
