Description

Community project funding/congressionally directed spending - construction - over the past few years, fears related to immigration status (ICE raids, public charge), the dismantling of the affordable care act (ACA) and covid-19 has caused community members to stop seeking essential preventive and chronic disease services (medical, dental and behavioral health). Disenrollment from health plans and safety net programs has increased throughout CA-35 (inland empire and pomona valley) and access to care has significantly decreased. Gains in outcomes and health equity made because of the ACA have fallen. To return to a post-pandemic healthcare environment, parktree will utilize a mobile van program to RE-engage people in need of healthcare services, assist with enrollment in payor programs and provide necessary medical, dental and behavioral healthcare to adults and children. Parktree will "drive" throughout the community, work collaboratively with schools, community-based organizations, homeless shelters, religious and spiritual groups and civic leaders to reeducate the community on healthcare access, utilization and to provide direct care services.
