THEY WILL PROVIDE TRANSPORTATION AND SOCIALIZATION, GIVE ELDER JUSTICE PRESENTATIONS TO GROUPS OF OLDER ADULTS, AND MEET INDIVIDUALLY WITH OLDER ADULTS THROUGH A NETWORK OF 2 STATIONS INCLUDING THE FAITH IN ACTION PROGRAM (PREVIOUS NEIGHBORHOOD OUTREACH PROGRAM )OF ERAS SENIOR NETWORK AND THE MILWAUKEE COUNTY DEPARTMENT ON AGING. THE PRIMARY FOCUS AREA OF THIS PROJECT IS HEALTHY FUTURES. THIS APPLICATION ALSO ADDRESSES THE CNCS FUNDING PRIORITIES OF AGING IN PLACE -- ELDER JUSTICE, AND AGING IN PLACE - INDEPENDENT LIVING. AT THE END OF THE THREE-YEAR GRANT, 420 OLDER ADULTS WILL BE SERVED AND 350 OLDER ADULTS WILL HAVE INCREASED SOCIAL SUPPORT OR IMPROVED CAPACITY FOR INDEPENDENT LIVING.
