CDD IS APPLYING FOR SIXTEEN VOLUNTEER SERVICE YEARS AND $75,352 IN FEDERAL FUNDING AND EIGHT VOLUNTEER SERVICE YEAR AND $56,230 IN STATE FUNDING, TO SUPPORT THE SENIOR COMPANION PROGRAM. THE SERVICE CATEGORIES THE PROJECT WILL ADDRESS ARE COMPANIONSHIP/OUTREACH AND IN-HOME/RESPITE. THE PROGRAM WILL USE PROJECT STAR WORK PLANS WITH PERFORMANCE MEASUREMENT TOOLS SUCH AS CLIENT AND CAREGIVER INTERVIEWS AND COMMUNITY PARTNER SURVEYS. THE COMPANIONSHIP/OUTREACH WORK PLAN ADDRESSES COMMUNITY NEEDS AND THE CORPORATION'S STRATEGIC PLAN; HEALTHY FUTURES, BY ASSISTING CLIENTS TO REMAIN IN THEIR HOMES, PROVIDING INFORMATION AND ACCESS TO COMMUNITY RESOURCES, AND PROVIDING OR ARRANGING TRANSPORTATION TO PRIMARY AND PREVENTATIVE HEALTH CARE APPOINTMENTS.
