EXECUTIVE SUMMARY SHEPHERD'S CENTER OF KC CENTRAL (SCC) EMPOWERS MID-LIFE AND OLDER ADULTS TO LIVE HEALTHY, ENGAGED, AND INDEPENDENT LIVES. IT HAS ADMINISTERED THE SENIOR COMPANION PROGRAM (SCP) IN THE KANSAS CITY, MISSOURI AREA FIRST AS THE WESTPORT COOPERATIVE CENTER AND LATER AS THE SCC SINCE 1981. SCC IS REQUESTING FUNDING TO CONTINUE ITS MISSION AND SPONSORSHIP OF THE SCP. AN ESTIMATED 69 SENIOR COMPANION VOLUNTEERS WILL SERVE HOMEBOUND SENIOR AND DISABLED CLIENTS. OF THIS NUMBER, ALL WOULD BE PLACED IN OUTCOME ASSIGNMENTS. THEIR ACTIVITIES WILL INCLUDE NOT ONLY COMPANIONSHIP AND RESPITE, BUT ALSO ASSISTING SENIORS AND PERSONS WITH DISABILITIES IN THE ACTIVITIES OF DAILY LIVING IN ORDER FOR THEM TO CONTINUE TO STAY IN THEIR OWN HOMES. THE PRIMARY FOCUS AREA OF THIS PROJECT IS HEALTHY FUTURES AND AGING IN PLACE, ACHIEVING SCC'S GOAL OF EMPOWERING SENIORS AND PERSONS WITH DISABILITIES TO REMAIN IN THEIR OWN HOMES WITH THE SAME OR IMPROVED QUALITY OF LIFE FOR AS LONG AS POSSIBLE. SCC SEES OUR SENIOR COMPANIONS AS FIRST ALERT ACTIVISTS WHEN THE POPULATION THEY SERVE MAY NEED HELP OR SERVICES SCC CANNOT SUPPLY BUT MAY BE AVAILABLE IN OUR COMMUNITY. AT THE END OF THE THREE YEAR GRANT SCC ANTICIPATES FULFILLING ITS MISSION LISTED ABOVE 100%. THE PROPOSED CNCS INVESTMENT OF $316,435 WOULD BE SUPPLEMENTED BY $35,164 IN NON-FEDERAL RESOURCES.
