SOME OF THEIR ACTIVITIES WILL INCLUDE VISITING WITH INDIVIDUAL CLIENTS, PROVIDING MUSIC AND MEMORY ACTIVITIES TO CLIENTS, ACCOMPANYING CLIENTS TO MEDICAL APPOINTMENTS OR SOCIAL ACTIVITIES, AND PREPARING LIGHT MEALS FOR CLIENTS. THE PRIMARY FOCUS AREA OF THIS PROJECT IS HEALTHY FUTURES. AT THE END OF THE THREE-YEAR GRANT, AT LEAST 60 HOMEBOUND OR OLDER ADULTS AND INDIVIDUALS WITH DISABILITIES WILL REPORT HAVING INCREASED SOCIAL TIES/PERCEIVED SOCIAL SUPPORT AND AT LEAST 48 CAREGIVERS OF HOMEBOUND OR OLDER ADULTS AND INDIVIDUALS WITH DISABILITIES WILL REPORT HAVING INCREASED SOCIAL TIES/PERCEIVED SOCIAL SUPPORT.
