THE DEKALB COUNTY BOARD OF HEALTH?S (BOARD OF HEALTH) MISSION IS TO PROTECT, PROMOTE, AND IMPROVE THE HEALTH OF THOSE WHO WORK, LIVE, AND PLAY IN DEKALB COUNTY. THE PROPOSED VISTA PROJECT ALIGNS WITH THE HEALTHY FUTURES FOCUS AREA. THE VISTA PROJECT WILL ASSIST IN PROMOTING COMMUNITY COLLABORATION AND BUILDING CAPACITY THROUGH OUR NUTRITION/FOOD SYSTEMS, COMMUNITY-CLINICAL LINKAGES, AND INJURY PREVENTION INTERVENTIONS. WE EXPECT TO IMPROVE HEALTH OUTCOMES IN AFRICAN-AMERICAN COMMUNITIES IN DEKALB COUNTY CENSUS TRACTS WITH AT LEAST 20% OF THE RESIDENTS LIVING BELOW 100% OF THE FEDERAL POVERTY LEVEL. THESE TRACTS ARE HOME TO 30% OF THE COUNTY?S POPULATION. FIVE VISTA MEMBERS WILL CONTRIBUTE TO THE GOALS OF THE PROJECT BY PERFORMING THE FOLLOWING ACTIVITIES: (1) IDENTIFYING AND SECURING FUNDING TO IMPLEMENT THE HEALTH BUCKS NUTRITION INCENTIVE PROGRAM. (2) DEVELOPING A COMMUNITY HEALTH WORKER PROGRAM TO SUPPORT ESTABLISHING PARTNERSHIPS WITH COMMUNITY-BASED ORGANIZATIONS AND AGENCIES FOR THE DEVELOPMENT OF A CLOSED-LOOP REFERRAL NETWORK THAT LINKS THE CLIENTS TO HEALTH-FOCUSED AND OTHER RELATED SUPPORT SERVICES. (3) CONTINUING TO SUPPORT THE PREVIOUS INJURY PREVENTION INITIATIVES, INCLUDING THE PEDESTRIAN AND CAR SEAT SAFETY PROJECTS, YOUTH VIOLENCE PREVENTION INITIATIVE, AND MENTAL HEALTH PROJECTS. THE VISTA MEMBERS? RESPONSIBILITIES WILL INCLUDE PROVIDING ASSIGNMENT RELATED ADMINISTRATIVE SUPPORT (I.E., FOLLOW-UP PHONE CALLS AND EMAILS RELATED TO THE PROJECT AND STRENGTHENING THE CAPACITY TO INCREASE COMMUNITY ENGAGEMENT. THESE ACTIVITIES WILL BE PERFORMED OVER THREE YEARS.
